It is used to sever the periodontal ligament (PDL) from the root surface of the tooth. The blades are pushed into the depth of the sulcus to sever the PDL around the circumference. It works when pushed several millimeters into the PDL and repeated space until reaching the apex’s distance. How Periotomes Work?
It is held in a modified pen grasp and inserted at 20 degrees to the tooth’s long axis into the gingival sulcus. It is used to sever the cervical gingival attachment fibers first and then several millimeters. Once the access was obtained, the instrument was gradually advanced into PDL space, repeating the same motion, and then tooth extracted using forceps exerting rotational force in a coronal direction.
